You must send your notification by email or submit it online

You must tell us about the death of a person using your service if either of the following has happened:

  • the person died while a regulated activity was being provided
  • their death may have been a result of the regulated activity or how it was provided.

You must notify us as soon as possible.

There is a specific notification form to use for the death of a detained mental health patient.

NHS trusts: report deaths through NRLSor LFPSE. They do not need to notify us separately.

How to notify us

There are 3 ways you can tell us about a death.

Use our existing provider portal

If you already have an account you can notify us online through the CQC Provider Portal.

Email a Word form

Use this form to tell us about the death of a person using your service.

Email the completed form to HSCA_notifications@cqc.org.uk.

Your notification

Your acknowledgement number may be different to what you expect. As we upgrade our systems you will receive either an ENQ reference number, notification ID, or case ID. Any of these numbers will identify your notification.
