Death of a person using the service – notification form
Submitting notifications during the coronavirus (COVID-19) outbreak
You must send your notification by email or submit it online. Do not send it by post. This is because our offices are closed and our staff are working from home.
You must tell us about the death of a person using your service if either of the following has happened:
- the person died while a regulated activity was being provided
- their death may have been a result of the regulated activity or how it was provided
You must tell us about the death without delay.
If you have an account with the CQC Provider Portal, you can send us your notification online.

Use this form to tell us about the death of a person using your service.
This form has been updated to include coronavirus (COVID-19) as a cause of death. The form also asks where the death occurred.
Send us the form
email the completed form to HSCA_notifications@cqc.org.uk.
NHS trusts report deaths through NRLS. They do not need to notify us separately.
