When you need to tell us about a death
You must notify us when:
- a person has died while being detained (or liable to be detained) under the Mental Health Act 1983
You must notify us of the death as soon as possible.
If the death is related to a patient safety incident, you should also report to your local risk management system (LRMS). Use the LRMS report number when you notify us so we can identify the duplicate reports.
