Changes to registered details: nominated individuals, officers and directors – notification form
Submitting notifications during the coronavirus (COVID-19) outbreak
You must send your notification by email. Do not send it by post. This is because our offices are closed and our staff are working from home.
When and how to tell us if your organisation's nominated individual, officers or directors are going to change.
How to notify us
Please tell us by filling in this form (sections 1 and either 7 or 8) and emailing it to HSCA_notifications@cqc.org.uk.
When you need to tell us about changes to the nominated individual, officers or directors
You must notify us if:
- you appoint a new nominated individual, or
- a director, secretary or other officer joins or leaves your organisation.
Please tell us if a new chief executive joins your organisation using section 8 of the form (this is not a statutory requirement).
