Submitting notifications during the coronavirus (COVID-19) outbreak
You must send your notification by email. Do not send it by post. This is because our offices are closed and our staff are working from home.
When and how to tell us if a service provider becomes insolvent.
How to notify us
Please tell us by filling in this form (sections 1 and either 9 or 10) and emailing it to HSCA_notifications@cqc.org.uk.
When you need to tell us about insolvency
You must notify us if you are a service provider and:
- a trustee in bankruptcy, a receiver, liquidator or provisional liquidator is appointed, or
- an individual's estate is sequestrated.
