Absence of a registered individual for 28 days or more – notification form
Submitting notifications during the coronavirus (COVID-19) outbreak
You must send your notification by email or submit it online. Do not send it by post. This is because our offices are closed and our staff are working from home.
When and how to tell us if a registered person or registered manager will be absent from their service for 28 consecutive days or more.
How to notify us
Notify us by filling in this form and emailing it to HSCA_notifications@cqc.org.uk
You must tell us when the registered individual has returned after absence.
When you need to tell us about a registered individual's absence
You must notify us if a registered manager or a registered person is going to be absent for 28 consecutive days or more.
You must submit your notification within these timescales:
- planned absence: 28 days before it starts (contact us at HSCA_notifications@cqc.org.uk if 28 day's notice is not possible)
- emergency absence: within 5 working days of the start
