The registered provider must ensure that the regulated activity Transport services, triage and medical advice provided remotely is managed by an individual who is registered as a manager in respect of that activity at or from all locations.

Condition of this registration relating to carrying out this regulated activity

The registered person must ensure that an effective written prescribing policy is implemented to support safe prescribing. This policy must be reviewed by a GMC registered General Medical Practitioner with a license to practice responsible for clinical governance and oversight at White Pharmacy Ltd and must include reference to all elements described in the condition as listed below. This must be submitted to CQC for review and implemented by 15 June 2017.

The registered person must assure themselves that, at the time of prescription

of any medicine detailed in the British National Formulary Sections [4.7.1,

4.7.2, 4.7.3, 4.8.1], the prescribing clinician:

• EITHER: Assure themselves that a patient is registered with a General Practitioner and that consent is given for information sharing with that practitioner (or another practitioner in that practice). Where there is insufficient or insufficiently reliable information given by the patient in order for them to prescribe safely, the prescribing clinician must clarify the information with the registered GP. In all circumstances any subsequent prescribing of medicines must be communicated to the registered GP.

• OR: Where a patient does not consent to information sharing with their GP or they are not registered with a GP, fully document the clinical rationale of why the prescription, including its volume, is appropriate despite being unable to share information with a General Practitio

The registered provider must notify the Care Quality Commission of any changes or amendments to the implementation of their voluntary measures outlined in their response email to CQC on the 1 June 2017.

Terms of this registration relating to carrying out this regulated activity

The registered person must take into account the ‘Good practice in prescribing and managing medicines and devices’ Guidance produced by the General Medical Council when deciding how to protect patients from the risk of unsafe prescribing of medicines (including opioid based medicines) at White Pharmacy Ltd.

Where patients are registered with a general practitioner, in order to ensure safe care and treatment is provided to the patient overall, the registered person must decide whether they are able to treat patients who refuse to give consent to White Pharmacy Ltd for their general practitioner to be contacted and informed of the medicines (including opioid based medicines) that have been prescribed.

The registered provider must ensure that the regulated activity Treatment of disease, disorder or injury is managed by an individual who is registered as a manager in respect of that activity at or from all locations.
