During an inspection looking at part of the service

We observed that the ward was busy, with people using the service either engaged in activity with their peers or in discussion with staff and social workers. The atmosphere was calm, and interaction between people using the service and staff appeared to be good.

People using the service told us that they were well cared for and that, "staff are kind and they talk to me." "Visitors are welcome at any time." "I know who my key staff are and get to talk to them often." They told us that they were included in discussions about their care and had been involved in care planning. We confirmed this from a sample of care records.

We asked people using the service if they saw a doctor regularly. One person said, " I see a doctor about twice a week, but I can see one when I want to." Other people confirmed this.

We observed some people using the service engaging in table top craft activities, we were told that a number of occupational activities were being provided. People said, "I enjoy the activities they provide and try to go to alot of the sessions." Another said, "You can be really busy if you go to all the things they provide."

We observed a mealtime and people told us that the food was okay, but they didn't like some of the food choices available and felt their was too much repetition in the menu's.

People told us if they had any concerns they felt able to talk to staff about them, we confirmed that information is provided detailing how people can make a complaint if they need to.

During an inspection in response to concerns

No concerns were identified relating to patient welfare or well being by LINK, or from Shropshire PCT. One of the two lead PCT commissioners of the Trust raised general concerns about how the trust manages and reports safeguarding concerns. The local authority safeguarding team, local coroner and our mental health act commissioner noted their shared concerns on how the trust had responded to previous incidents and issues raised about Shelton Hospital.

During the site visit we visited two wards, the occupational therapy department, and the advocacy team and met with the divisional director. All patients we spoke to described feeling safe on the ward and no concerns were raised. They were happy with their bedrooms and felt they had sufficient privacy and freedom to be in their bedroom during the day. They felt the standards on the wards were good and that staff were always accessible but not intrusive.

One person said, 'I don't know how staff manage, they are really patient and always make time for me. They have supported me when I've been really anxious and I know feel prepared for my discharge.'

Another said, 'The staff are really approachable, and I get to meet with my named nurse when I want to. She talks to me about my care plans and I have been involved in reviews.'
