About the service

Rosevilla Nursing Home is a nursing home providing personal and nursing care to up to 49 people. The service provides support to older people some of whom were living with dementia. At the time of our inspection there were 35 people using the service.

People’s experience of using this service and what we found

People’s care plans were not always up to date, and safety monitoring was not in place for 1 person’s complex needs. The provider had made improvements at our last inspection, however we found further improvements were still required to ensure people received effective and safe care.

People were supported by staff who were trained to recognise and report on the risk of abuse. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

People were supported by enough staff who were safely recruited to work at the home. People’s medicines were managed effectively. Effective infection control practices were in place. Lessons were learnt when things went wrong.

Staff and managers shared a positive culture, and they demonstrated a passion to provide improved care to people. The provider acted on the duty of candour under their legal responsibility. People, their relatives and staff were involved in the running of the service. The provider worked well with other agencies to help achieve good outcomes for people.

The last rating for this service was requires improvement (published 26 September 2022).

Why we inspected

We received concerns in relation to people’s nursing care needs and the quality assurance systems in place. As a result, we undertook a focused inspection to review the key questions of safe and well-led only.

For those key questions not inspected, we used the ratings awarded at the last inspection to calculate the overall rating. The overall rating for the service has remained the same based on the findings of this inspection.

We have found evidence that the provider needs to make improvements. Please see the safe and well-led sections of this full report.

We looked at infection prevention and control measures under the Safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively.

During a routine inspection

About the service

Rosevilla Nursing Home is a nursing home providing personal and nursing care to up to 49 people across two floors. There are multiple communal areas at the service and an accessible outdoor space. The service provides support to older people. At the time of our inspection there were 25 people using the service some of which were living with dementia.

People’s experience of using this service and what we found

Whilst there had been significant improvements to the quality assurance tools at the service and the quality of care, further improvements were ongoing to ensure changes were embedded into staff practice.

People felt safe and able to raise concerns about their care should they have them. There were enough staff to meet people's needs in a timely way. People were supported to minimise their risks by staff who knew them well. People were supported in a clean environment. People received their medicines as prescribed.

People were supported to eat and drink in line with their needs. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

People were supported by kind and caring staff. People told us they were not rushed, and staff maximised their dignity and independence.

People gave positive feedback about the management team. People had access to health professionals where they required them.

Rating at last inspection and update

Our last inspection was a focused inspection where we rated this service inadequate in safe and requires improvement in well led (published 05 February 2022) and there were breaches of regulations. We told the provider our concerns and when they had to be compliant with regulations by. The provider completed action plans after the last inspection to show what they would do and by when to improve.

At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

This service was registered with us on 01 May 2020 and this is the first comprehensive inspection.

Why we inspected

This inspection was carried out to follow up on action we told the provider to take at the last focussed inspection. The overall rating for the service is now requires improvement.

About the service

Rosevilla Nursing Home is a residential care home which provided personal and nursing care to 35 people at the time of our inspection.

The service can support up to 49 older people, younger people and people living with dementia and physical disability in one adapted building. The service provides a ‘Discharge to Assess’ service which provides support to people who have been discharged from hospital to enable them to go back to their own homes.

People’s experience of using this service and what we found

Risks to people's health and wellbeing had not always been identified and assessed to ensure they were managed safely. Staffing was not sufficient to ensure people were supported in a timely way. Shortfalls in staffing were covered by the use of agency staff. Systems for ensuring people received their medicine as prescribed needed improvement. People were not always protected against the risk of cross infection within the home.

Current government guidance in relation to keeping people safe during the COVID-19 pandemic was not always followed by staff. The provider’s own policies were not always followed for infection control, vaccination as a condition of deployment and admissions into the home. People’s care records did not always evidence the support they had received. The provider’s oversight and governance had not been effective in identifying and rectifying areas of concern.

Why we inspected

We received concerns in relation to the management of risk. As a result, we undertook a focused inspection to review the key questions of safe and well-led only.

We looked at infection prevention and control measures under the safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively.

We reviewed the information we held about the service. No areas of concern were identified in the other key questions. We therefore did not inspect them. Ratings from previous comprehensive inspections for those key questions were used in calculating the overall rating at this inspection.

The overall rating for the service has changed from good to requires improvement. This is based on the findings at this inspection.

Enforcement

We are mindful of the impact of the COVID-19 pandemic on our regulatory function. This meant we took account of the exceptional circumstances arising as a result of the COVID-19 pandemic when considering what enforcement action was necessary and proportionate to keep people safe as a result of this inspection. We will continue to discharge our regulatory enforcement functions required to keep people safe and to hold providers to account where it is necessary for us to do so.

We have identified breaches in relation to the management of risk, infection prevention and control, staffing and governance at this inspection.

Follow up

We will request an action plan for the provider to understand what they will do to improve the standards of quality and safety. We will work alongside the provider and local authority to monitor progress. We will return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.
