During an inspection looking at part of the service

We carried out an inspection of this service following our annual review of the information available to us including information provided by the practice. The practice was previously inspected on 12 June 2017 and we rated the practice as Good overall.

Due to the assurance received from our review of information, we carried forward the good ratings for the following key questions: Safe, Caring and Responsive. This inspection looked at the following key questions:

  • Are services Effective
  • Are services Well-Led

We based our judgement of the quality of care at this service on a combination of:

  • What we found when we inspected
  • Information from our ongoing monitoring of data about services and
  • Information from the provider, patients, the public and other organistions

We have rated this practice as good overall and good for all population groups with the exception of families, children and young people, which has been rated as requires improvement.

At this inspection we found:

  • Patients received effective care and treatment that met their needs.
  • The culture of the practice and the way it was led and managed meant the delivery and continual improvement of quality person-centred care.

Whilst we found no breaches of regulation, the provider should:

  • Continue with efforts to increase the up-take of child immunisations and cervical screening.

Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Clayhall Clinic on 28 September 2016. The overall rating for the practice was Good. The full comprehensive report on the September 2016 inspection can be found by selecting the ‘all reports’ link for Clayhall Clinic on our website at www.cqc.org.uk.

At our previous inspection on 28 September 2016, we rated the practice as Requires Improvement for providing safe services, as the practice had not ensured that there was adequate hand washing facilities in the staff toilet to prevent the spread of infection.

This inspection was an announced focused inspection carried out on 12 June 2017 to confirm that the practice had carried out their plan to meet the legal requirements in relation to the breaches in regulations that we identified in our previous inspection on 28 September 2016. This report covers our findings in relation to those requirements. At this inspection, we found that the practice had addressed our concerns and carried out their legal requirement in relation to the breach in regulation by having a wash basin installed in the staff toilet. As a result of these findings, the practice is rated Good for providing safe services. The overall rating remains as Good overall.

Our key findings were as follows:

  • The practice had installed washing facilities within the staff toilet to help prevent the spread of infection within the practice.

We carried out an announced comprehensive inspection at Clayhall Clinic on 28 September 2016. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • There was an open and transparent approach to safety and an effective system in place for reporting and recording significant events.
  • Staff assessed patients’ needs and delivered care in line with current evidence based guidance. Staff had been trained to provide them with the skills, knowledge and experience to deliver effective care and treatment.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available and easy to understand. Improvements were made to the quality of care as a result of complaints and concerns.
  • Patients said they were always able to make an appointment and urgent appointments were always available on the same day.
  • There was a clear leadership structure and staff felt supported by management.
  • The practice proactively sought feedback from staff and patients, which it acted on.
  • The provider was aware of and complied with the requirements of the duty of candour.

The areas where the provider must make improvement are:

  • Ensure adequate hand washing facilities are provided in the staff toilet to prevent the spread of infection.

The areas where the provider should make improvement are:

  • Consider providing baby changing facilities for the patients
  • Review how patients with caring responsibilities are identified and recorded on the clinical system to ensure information, advice and support is made available to them.
  • Review care plans to ensure they contain enough information about treatment, care and support that was being delivered.
  • Ensure written consent is sought when minor surgery is carried out.

During a routine inspection

We spoke to people visiting the surgery on the day of our inspection. People told us they were happy with the service provided by the GP at the practice. One person said, 'they are good here, I can always get an appointment.' Another person said, 'I never had a problem with anything, my doctor is ok.'

We found that people's care was planned and delivered in a way that met their individual needs and that the practice co-operated with other healthcare professionals and services.

We also found that people were protected from the risk of abuse because the provider had procedures in place for safeguarding vulnerable adults and children, staff we spoke with were aware of these procedures.

People we spoke with told us that they were always able to get an appointment quickly and did not have problems getting an emergency appointment.

We found that people's privacy and dignity was respected.

The provider had effective recruitment procedures in place to ensure only suitable staff were employed at the service.
