Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Colne Road Surgery on 09/09/2015. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • Staff understood and fulfilled their responsibilities to raise concerns, and to report incidents and near misses. Information about safety was recorded, monitored, appropriately reviewed and addressed.
  • Risks to patients were assessed and well managed, however were not fully recorded.
  • Patients’ needs were assessed and care was planned and delivered following best practice guidance. Staff had received training appropriate to their roles and any further training needs had been identified and planned.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available and easy to understand.
  • Patients said they found it easy to make an appointment with a named GP and that there was continuity of care, with urgent appointments available the same day.
  • The practice had good facilities and was well equipped to treat patients and meet their needs.
  • There was a clear leadership structure and staff felt supported by management. The practice proactively sought feedback from staff and patients, which it acted on.

However there were areas of practice where the provider needs to make improvements.

Importantly the provider should;

  • Ensure all risk assessments are fully documented.
  • Ensure the Locum pack is available to all locums working at the practice.

During an inspection looking at part of the service

We undertook a visit in order to follow up on action taken by the practice, after concerns were found at the last inspection in December 2013. This was in relation to the storage of records and guidance for staff on record keeping.

We found there had been significant improvements made in how the practice managed records and record keeping.

We found staff had appropriate policy guidance on how to create, manage and store confidential information.

We saw records were stored securely.

During an inspection in response to concerns

We visited the practice as we had received concerns about the cover/on call arrangements between the GPs, cleanliness of the practice and the storage of patient records. We discussed these concerns with the local NHS England primary care team.

During our visit we spoke with one of the GPs, the practice manager, one receptionist and seven patients. We were unable to speak with the registered manager/GP.

One of the GPs and with the practice manager told us agreements were in place that should help to improve the on call cover arrangements and working relationships between the GPs.

Patients told us they were happy with the service and the treatment they received. They told us they were fully involved in discussions and decisions about their treatment and said they were listened to. Patients said they had no difficulties booking an appointment.

We found the building was old and in need of refurbishment. For example the carpets in the waiting area and corridor were in need of replacement as they were worn and stained. Staff had access to up to date training and policies and procedures relating to infection control.

We found patients' confidential records were not stored securely, were stored in a number of places in the practice and therefore could not be located promptly when required. Patients' records were not kept in accordance with legislation. We asked the provider to address these shortfalls and we will check to see that action has been taken.
