Recovery plans also included individual risk assessments with an associated risk management plan containing clear information regarding how the risk management plan could be implemented. This included information on harm reduction. For example information on the potential synergy of illicit substances on prescribed medication.

We saw meeting and review records that demonstrated staff supporting people who used the service regularly contacted other relevant agencies involved in people's care and support; for example social workers, GPs and community psychiatric nurses (CPNs).

Multi-Disciplinary Team (MDT) meeting minutes demonstrated that all local safeguarding issues were discussed on a weekly basis. We also saw documents which confirmed that all safeguarding issues were regularly monitored by the provider's Clinical Incident Review Group (CIRG), safeguarding lead and Quality Audit & Governance Manager for West Sussex.

Appropriate procedures were in place for the secure storage of blank prescription forms and other related stationery. There were tight controls and completed records of all prescription forms used. Details were kept of each prescription and any that were destroyed. This meant that there would be no misappropriation.

We observed that there were sufficient staff throughout our visit to provide services to people attending their appointments. We saw that people who used the service were greeted respectfully and with courtesy. The atmosphere in the service appeared to be relaxed and friendly and we observed staff being caring and supportive in their approach to people.

We saw that the service had regularly gained the views and experiences of people who used the service in the way the service was provided and delivered. A yearly survey had been completed by the provider. The outcome of the survey had been published in the provider's service user newsletter and was also fed back to people through the provider's two service user forums.
