We saw they were developed and reviewed with the person using the service and /or their relatives where appropriate. They included detailed and signed consent to treatment forms.

People who used the service were protected from the risk of abuse, because the provider had taken reasonable steps to identify the possibility of abuse.

The records were kept securely. They were well organised and detailed.
