During a routine inspection

We visited the service on the 11 July 2013 to follow up concerns we first identified on the 9 April 2013 in relation to care and welfare of people using the service, staffing and record keeping. The provider sent us an action plan stating how they were going to improve the service to ensure the welfare and safety of people using it.

During our inspection on the 11 July we found that records had improved and clearly told us how staff were meeting people's needs. We observed high levels of care being provided by staff. People were given good care and monitored regularly for their safety.

The provider had recruited additional staff and there were no reported vacancies. However on the day of inspection we identified staffing shortages, which were covered later the same day. We found that staff were working hard but reported high levels of stress, partly attributed to on-going staff shortages within the service. We also identified issues with the level of support that staff were being provided with, which was inadequate.

During a routine inspection

We spoke with six people using the service one relative, the district nurse and six staff during our inspection. One person was celebrating their hundred birthday and a party was planned for the afternoon. Their life had been spent with horses and one of the relatives brought in a horse for them to see.

The service was clean and well maintained and provided spacious accommodation.

One person told us 'It's lovely, what more could you want? Coffee, TV and it is warm. Food is lovely and you get a good choice of food.' Another person told us 'Everything is fine. Staff are very nice, I have no complaints. There is no real choice of food, I just eat what I am given."

We inspected five people's care plans which told us what people's needs were but they had not been updated when people's needs had changed, so we could not be assured that care was planned appropriately.

On the day of inspection there were not enough staff to meet people's needs because of staff shortages on both shifts. Staff told us people did not get the input they required and did not have their social needs appropriately met.

There were effective recruitment processes in place and staff received regular training. Staff did not receive regular supervision and formal support so we could not see how the provider monitored their performance.

There were safe systems for the storage and administration of medication which meant people were protected from the risks of unsafe administration.
