During an inspection looking at part of the service

About the service

Edwardstow Court Care Centre is a care home without nursing providing care to 34 people aged 65 and over at the time of the inspection. The service can support up to 48 people.

People’s experience of using this service and what we found

We undertook this targeted inspection to check on a specific concern we had about medicines, falls and safeguarding.

People were supported by staff who understood how to keep them safe from the risk of abuse. Appropriate safeguarding procedures were in place and staff had been trained in how to protect people from abuse.

People living in the service received personalised care around their medicines. The provider was taking action to ensure information about people’s medicine needs and action taken in response to medicine audits would always be recorded. The Medicine Quality Lead was supporting the home and a pharmacy audit was taking place in November to further review medicine practices.

There were systems in place to monitor risks associated with falls. Staff had received training in falls prevention and the registered manager routinely monitored any patterns. They were developing the way in which they recorded actions taken and required to further strengthen this process.

The registered manager was continually reviewing the challenges they experienced with recruitment and retention to minimise the impact on people. Additional support had been put in place to support agency staff and agency staff told us the investment from management had been beneficial. One member of agency staff said, “It’ a nice home. As soon as a new member of agency staff comes in the registered manager gives a walk around and explains everything, all their policies and procedures.” Staff were positive about how the Registered Manager was addressing the challenges. One staff member told us, ‘‘The registered manager manages well to ensure that staff provide safe and personalised care.”

The provider had increased their support and oversight of the home to ensure that any actions identified by the registered manager and quality audits were completed and that people remained safe whilst improvements were being made.

Why we inspected

We undertook this targeted inspection to check on a specific concern we had about medicines, falls and safeguarding. The overall rating for the service has not changed following this targeted inspection and remains good.

We use targeted inspections to follow up on Warning Notices or to check concerns. They do not look at an entire key question, only the part of the key question we are specifically concerned about. Targeted inspections do not change the rating from the previous inspection. This is because they do not assess all areas of a key question.

We looked at infection prevention and control measures under the Safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively.

During a routine inspection

About the service

Edwardstow Court Care Centre is a ‘care home’ providing personal and nursing care to 33 people aged 65 and over at the time of the inspection. The service can support up to 48 people.

People’s experience of using this service and what we found

The service was outstandingly responsive to people's individual belief's, preferences and needs, and people’s wellbeing was enhanced by person-centred activity planning. The activities co-ordinator used national guidance to support activity planning and ensure activities were tailored to the interests, needs and hobbies of the people living at the service.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

The home was well led by a management team who were passionate about providing high quality individualised care to people. The registered manager and deputy manager led by example and without exception staff told us that they were well supported.

Robust quality assurance systems and a strong learning culture contributed to continual development and improvement of the home. The registered manager told us, “The journey has been amazing but we are still on the journey, and the journey will never end.”

There was a culture of listening to people and positively learning from events so similar incidents were not repeated. As a result, the quality of the service had continued to grow and develop. Feedback about the service from people and those close to them was consistently positive, whilst feedback from other professionals indicated effective partnership working.

All staff demonstrated extremely compassionate, attentive and caring approaches in their interactions with people. They consistently promoted and encouraged people's independence and treated them with dignity and respect. People and their relatives were complimentary about the approach of the staff and the management team. Everybody we spoke with said they would recommend the service.

People were supported by staff who understood how to keep them safe from the risk of abuse. Appropriate safeguarding procedures were in place and staff had been trained in how to protect people from abuse.

Support of the staff team was paramount to the registered manager, who told us, “We are nothing without the team.” The staff team worked as one to provide high quality, personalised care to the people living in the service. One staff member said, “We are such a strong team. We know people’s strengths and areas of development, and we all complement each other well.”

We undertook this inspection as part of a random selection of services rated Good and Outstanding to test the reliability of our new monitoring approach

About the service: Edwardstow Court Care Centre is a ‘care home’. Edwardstow Court Care Centre opened in 2018 and can accommodate 48 people. At the time of our inspection 20 people aged 65 and over lived at the home.

People’s experience of using this service:

• People told us they felt safe; there were arrangements in place to protect them from potential abuse, discrimination and harm.

• People told us their health needs were met; health risks were identified and addressed and people had access to professionals who could support these needs.

• People’s medicines were managed safely and were available as prescribed.

• People told us there were enough staff to help them; staff were recruited safely and provided with training and support to work safely.

• People spoke highly of the care and compassion shown to them; they were treated with dignity, as individuals and their privacy was maintained.

• People told us they were supported to be independent and received explanations which helped them to make their own decisions.

• People unable to make independent decisions were protected from decisions which may not be in their best interests.

• People told us they had opportunities to make choices and express a view; people’s care was planned and delivered around their personal preferences.

• People’s representatives told us they were made welcomed and supported to be involved in their relatives’ care; where appropriate, they could act as an advocate for their relative.

• People told us there were opportunities for them to join in social and recreational activities and to remain part of the local community; staff protected people from the risk of isolation and disengagement from the wider community.

• People told us the environment they lived in met their needs and was comfortable; they had access to equipment which supported their needs, infection control measures were in place and the home was kept clean and safe.

• People and relatives told us that any areas of concern or dissatisfaction were addressed immediately; a complaints process was in place and managers were open to feedback which could be used to improve the service overall.

• People and their relatives knew who the managers of the home were; managers and senior staff were visible, approachable and involved in people’s day to day care.

• Quality monitoring processes ensured the service complied with necessary regulations and where needed, led to improvements which ensured people received a quality service.

All the feedback we received about the service from people and their representatives was positive. The service met the characteristics of ‘Good’ in all areas; more information is in the full report.

Rating at last inspection:

• This was the service’s first inspection; there has been no previous rating.

Why we inspected:

• We inspected this service as part of our ongoing Adult Social Care inspection programme. This was a planned inspection following the service’s registration with the Care Quality Commission in March 2018.
