Roman House is a residential care home providing accommodation and personal care to up to 8 people. The service provides support to people with a learning disability and autistic people. We carried out our on-site assessment on 10 January 2024 and 29 January 2024 ; off site assessment activity started on 12 January 2024 and ended on 1 March 2024. We looked at 5 quality statements; Safeguarding; Safe and effective staffing; Safe environments; Medicines optimisation and Governance, management and sustainability. We expect health and social care providers to guarantee people with a learning disability and autistic people respect, equality, dignity, choices and independence and good access to local communities that most people take for granted. 'Right support, right care, right culture' is the guidance CQC follows to make assessments and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it. There had been shortfalls with the leadership and governance of the service. This meant the safety and quality of people’s care had been compromised. We identified concerns about the management of medicines, staffing and the safety of the environment. A new manager had just started and was keen to make improvements. Senior leaders acknowledged the shortfalls and told us how they would work with the new manager to improve the service. There were effective safeguarding procedures and staff understood how to protect people from the risk of abuse.

The service was not always able to demonstrate how they were meeting the underpinning principles of Right support, Right care, Right culture Right Support: Care was not fully person-centred. For example, while staff knew people well and were caring in their approach, staffing levels impacted on the ability to always meet people’s needs and preferences. People were at increased risk of harm because their medicines were not always safely managed, and safety concerns were identified in the environment. Inconsistent leadership resulted in ineffective governance and quality assurance systems. Audits were not consistently completed, and actions were not followed up. This impacted on the care and support people received. Despite this, people told us they were happy and felt safe with the care and support they received at Roman House.
