During a routine inspection

Our inspection team was made up of two compliance inspectors, an expert by experience and specialist advisor. We spent the day at the home where we spoke staff, people using the service and their relatives. We also spent time observing practices and the interactions between staff and people using the service.

Staff commented that they felt the service was being well managed since the registered manager had been in post. Observations throughout the day showed that staff were busy but peoples care was not rushed. Relatives we spoke with were happy about the care their family members were receiving.

Along with our observations we looked at records of peoples care and the management of the home. Records seen showed that there had been improvements since the previous inspection but the home was still non-compliant with some outcomes.

During an inspection looking at part of the service

At our previous inspection of August 2012 we found concerns because there were not enough members of staff to keep people safe and meet their health and welfare needs.

The purpose of this inspection was to check compliance in this area and we found that significant improvements had been made.

In order to see how people were being supported we looked at people's care records and spoke with them about their experiences living at the home. Some people had a diagnosis of a dementia and were not able to tell us about their care so we spent time with them to help us understand what life was like at the home. To do this we used the SOFI.

During SOFI we noted an even balance of neutral and positive staff interactions. Neutral interactions were when staff undertook a task, for example giving someone a drink, but did not communicate or make eye contact with that person.

We spoke to three external professionals who visited the home to support people with their health or social care needs. All three expressed that they had no concerns about the level of staffing that they had seen when visiting.

We spoke with two relatives: one relative said "I like the way staff handle the residents". They told us that there were always enough drinks provided and that if there was 'anything wrong' they would always contact the home.

During a routine inspection

We carried out an inspection at Ferfoot care home one the 8 and 9 August 2012.

Some people were not able to tell us about their experience of living in the home because of limited communication or they were unable to express themselves verbally. However, we observed how people were cared for and talked to people who were able to express their views. One person told us, 'I am very well looked after, the staff are lovely'. We spoke with this persons' relatives during their visit and they told us they were, 'satisfied with the care given'.

During our inspection we found that some people were not being fully supported with their personal care and support to eat and drink. Most members of staff were respectful and treated people with dignity, however, we saw evidence on two occasions where people were not treated with respect. We observed there was a lack of social stimulation for people. We were told by the acting manager that the activities co-ordinator was off sick and the home did not have an alternative.

We talked to many staff during the two days and staff reported that they did not feel there were enough staff to support people appropriately. Staff also reported that they did not feel supported by the management team. They said the home lacked leadership due to the changes in management over the last few months.

We looked at the minutes of a relatives meeting held on 4 July 2012. It gave the responses given by relatives, 'lack of staff in lounges, for example, after tea and early evenings. 'The home is short of staff but staff worked very hard'.

We could see that people were comfortable with the staff in the home. We talked with

two people about being safe. They said they felt safe in the home and would tell the

staff if they had any problems with other residents or if they did not like something. One person said, "The staff are wonderful".
