During an inspection looking at part of the service

We found that the warning notices and compliance actions set at previous inspections were not met. We found that people who used the service still did not experience care, treatment and support that met their needs and protected their rights. The care records were not accurate and did not support staff to meet peoples' care needs. We found staffing levels were insufficient to ensure suitable observation levels and activities were provided.

We found that the standard of cleanliness in the home was inadequate and did not protect people from the risk of infection. Also the staff who were handling food had not received adequate levels of training to undertake this task.

People who used the service, staff and visitors were not protected against the risks of unsafe or unsuitable premises or the use of unsafe or unsuitable equipment. When we looked at the maintenance records we saw hand written information stating services had been completed but we could not confirm that these were accurate. Safe hot water temperatures were not maintained, which meant people were at risk of being scalded.

We found that the provider had not developed suitable arrangements to ensure that staff were supported to deliver care and support safely and to an appropriate standard.

However we did find that the care staff were dedicated to making sure people who used the service were well looked after and their care needs were met.

During a routine inspection

In this report the name of a registered manager appears who was not in post and at the time of the inspection was not managing the regulatory activities at this location. Their name appears because they were still recorded as Registered Manager on our register.

We found that people who used the service did not experience care, treatment and support that met their needs and protected their rights. The care records were not accurate and did not support staff to meet peoples' care needs.

People who used the service, staff and visitors were not protected against the risks of unsafe or unsuitable premises or the use of unsafe or unsuitable equipment. When we looked at the health and safety records we found that the last fire drill was documented as having been carried out on 21 March 2012. Also we found that safe hot water temperatures were not maintained, which meant people were at risk of being scalded.

We found that the provider had failed to implement appropriate pre employment checks to ensure that the people were cared for, and supported by suitably qualified, skilled and experienced staff. They had not developed suitable arrangements to ensure that staff were supported to deliver care and support safely and to an appropriate standard.

However, we found that the privacy, dignity and independence of people who used the service was respected

When asked how they found living at Acorn House one person said, "It's lovely".

During an inspection looking at part of the service

We found that people who used the service did not always have their privacy, dignity and independence respected. People's views and experiences were not always taken into account in the way that the service was provided and delivered in relation to their care. One person we spoke to told us 'When I first arrived it felt like home, it doesn't now and it hasn't for a while.'

Throughout the inspection we found that people who used the service did not experience care and treatment that met their needs. We found that records were not always available, appropriate and accurate which meant that people were not always protected from the risks of unsafe or inappropriate care.

We found that the provider could not demonstrate that there was an effective recruitment and selection process in place, ensuring that appropriate checks were undertaken before staff began work.

People who used the service, staff and visitors were not protected against the risks of unsafe or unsuitable premises and equipment.

People were protected from the risk of infection because the provider ensured that people were cared for in a clean and hygienic environment.

During an inspection in response to concerns

We used a number of different methods to help us understand the experiences of people using the service, because the people using the service had complex needs which meant they were not able to tell us their experiences. We spoke to staff and observed care being provided.

During a routine inspection

During our visit to the home people told us they were offered choices about the care they receive and how this was done. They also said that staff were very good. People

living at the home made comments such as, "The staff look after me well."

We talked to people about the care they received and everyone we spoke to during our visit to the home expressed satisfaction about the care at Acorn House. People

made comments such as "I feel I am pretty well looked after here." One person said "I am fine here. I often walk out to the local pub." We were told that the food is good and

plentiful.

People that we spoke to during our visit, told us "I'm happy with the care" and "Staff are

lovely." We were told that "The staff are good to me." One person said "I can go out when I want to and have a walk."

People told us that they were happy with the home and would speak to a member of staff if they had any concerns.
