During an inspection looking at part of the service

At a previous inspection on 22 November 2013, we identified moderate concerns with cleanliness and infection control and the management of emergency medications. The provider subsequently sent us an action plan to say how they were going to become compliant with the Health and Social Care Act (2008) Regulations 2010.

At this follow up inspection, we carried out checks to ensure the provider had met their action plan. We found the provider had made substantial improvements to the decontamination room and was now compliant with infection control guidelines. Medication within the practice was now appropriately stored and all emergency medication was in date.

During an inspection in response to concerns

We carried out an unannounced inspection on a Friday afternoon. The dental practice had two surgeries but only one was in use on the day of our visit. We spoke with two patients who had no concerns about the practice. They told us they had both been coming to the practice for many years and believed they were treated in a clean environment. They also told us they were asked to wear protective equipment during treatment. Comments included; 'I think the surgeries are clean. They are spotless' and 'The dentist always wears gloves and masks, and they cover me up.'

Guidance for the decontamination of instruments recommends that sterilised items are stored in bags and dated or if in daily use can be stored on trays that are covered. We found that dental instruments in one surgery were not covered or bagged. We also found in both surgeries that needles had been opened and attached to syringe barrels but there were no patients due in until the Monday morning. This meant the needles would not be sterile before use.

We looked at the drugs available for medical emergency situations. Three drugs we looked at within the emergency drug kit were out of date and a further two were due to expire in November 2013. We also found some of the equipment such as syringes was out of date. We found there were no checks in place to check the expiry dates which meant there was a risk that expired drugs could be given to a patient.

During a routine inspection

A patient using the service told us that they were happy with the service provided. They said they were able to discuss the treatment they were receiving and the costs involved with the dentist. They also told us that they were fully aware of the appropriate infection control procedures needed because of their job and there were no issues within the practice.

We noted that the practice appeared clean and well maintained. Dental nursing staff told us that they hold responsibility for cleaning all areas above the floor level and that there are arrangements in place to maintain hygiene standards throughout the practice.
