During an inspection looking at part of the service

In addition to reviewing care records and medicines administration records (MARs) we visited two people in their homes and spoke with staff about the administration of medicines. We spoke with one person about the administration of their medicines. They felt the system was reliable and they felt well supported. They played an active role in the administration of their medicines and understood the importance of telling staff what medicines they had taken. They told us they had been looked after by other services and this was the best one.

We found that work had been done to ensure the administration of medicines was reliably completed and recorded. Additional monitoring had been put in place to prevent errors going unnoticed. The MARs we looked at had been fully completed by staff. The staff administering the medicines felt confident about the systems in place. Some people had complex needs around medicines. Although staff knew how to meet these needs, care plans did not always contain enough information to ensure consistent support.

During an inspection looking at part of the service

We reviewed records and spoke with staff in order to verify whether the actions required of the provider following an inspection with non-compliance for medicines administration and notifications had been completed. We spoke with people about general topics but they were unable to comment on the submission of notifications or the administration of medicines.

We found that notifications of specific events had been sent to CQC as required. We found the recording of medicines administration had improved. We also found there was more information available to staff about the medicines they were administering. Staff had received further training in medicines administration. We also found, however, that one person had not received some medicines they were prescribed and some medicines recording was not accurate. The provider had not adequately investigated a failure to administrate medicines in order to prevent a re-occurrence.

During a routine inspection

The address in this report was no longer correct as the service had moved. The incorrect address appears because it was the address on our system at the time of the inspection. We have advised the provider how to change the address on our system.

The four people we spoke with and records we reviewed showed people received support that met their needs. One person told us 'they have made a huge difference ' it is a relief to be somewhere where people know how to help me'.

People told us they were encouraged to be as independent as possible and felt their individual preferences were respected. One person told us 'staff encourage me to do what I can by myself and help me with the rest'.

We found the company was following appropriate recruitment procedures and the necessary checks were in place to protect people receiving support.

The staff and people we spoke with were all familiar with how to raise a safeguarding concern. One person told us 'I can speak to staff if I'm worried'. Safeguarding concerns had been dealt with appropriately in the past although we had not been notified as required.

The system for recording medication administration was not robust. Staff did not have access to all the information they needed to administer medication safely.

The provider monitored service quality by speaking to people, observing staff and responding to incidents and complaints. People felt listened to and one person said 'all my previous issues have been sorted'.

During a routine inspection

We spoke with two people using the service and one relative of a person using the service. We also obtained feedback from people provided as part of the agency's quality assurance process.

People told us, "staff treat me with dignity and respect", "staff are fabulous" and "impressed by the caring vigilant team". One person said that staff supported them to remain independent where ever possible.

People said they had care records in their homes which had been discussed with them. They told us staff followed these to provide care and support. They said, "staff are amazing with me", "I can't fault them, they listen to me and have helped me out".

Feedback from health care professionals included, "the agency are very professional and approachable".

We did not receive any verbal feedback from people for this follow up review of compliance. Comments the provider received as part of their quality assurance process included:

"An excellent service."

"I am happy with the service."

"Staff are flexible."

"Really good carer."

"Happy as everything is at the moment."

During an inspection in response to concerns

People told us the service they received was 'excellent', 'staff understand our needs' and 'we couldn't ask for better'. They also said, 'the staff know me well. They arrive on time and help me twice a day.' We were told that staff were respectful and treated

people with dignity. One person said, 'my wife is unable to speak, staff talk to her and don't talk over her.' We were told, 'everything is documented. Staff have discussed my care plans with me.'

Other comments about the agency included, 'there has been a vast improvement in such a short space of time', 'provide an excellent service' and 'have helped with my independence'.

People told us they would talk to staff or the manager of the agency if they had any concerns. They said they found the management team and office accessible and approachable. People told us they had been given information about how to make a complaint and who to contact if they had any concerns.

People said, 'staff are excellent', 'they are well trained' and 'any issues are discussed and sorted quickly and amicably.'
