During a routine inspection

Before people received care and treatment they were asked for their consent and the provider acted in accordance with their wishes. We looked at support plans and found they indicated where people were able to make their own choice and where this had been respected. During our visit one person explained how they had chosen the colour of paint for their bedroom.

People's needs were assessed and care and treatment was delivered in line with their individual care plan. We spoke with people who used the service and they were happy with the care they received.

People were provided with a choice of suitable and nutritious food and drink. The support plans we looked at recorded people's nutritional needs. People told us that they were involved in menu planning, shopping and food preparation.

There was an effective recruitment and selection policy in place. Appropriate checks had been carried out prior to people commencing work.

People were made aware of the complaints system. There was a complaints procedure and people who used the service knew how to complain.

During an inspection looking at part of the service

We completed this inspection because our inspection of 13 February 2013, found that the service was not compliant with regulation 20 of the Health and Social Care Act. The provider wrote to us and told us they would be compliant by 30 April 2013.

This inspection also took place due to concerns raised with the Care Quality Commission in relation to staffing.

We found that people who used the service were supported by enough qualified, skilled and experienced staff to meet their needs.

We found that peoples' records were accurate and fit for purpose. Other records relating to the management of the service were also informative.

During an inspection in response to concerns

Many of the people who used the service could not tell us directly about their care due to a variety of complex needs. However, we spoke with two people who used the service and they told us that they enjoyed living at Rother Heights.

We observed interactions between staff and people who used the service. We found that staff knew how to relate to people and responded appropriately to their needs. However this was not always reflected within the care plan documentation.

We spoke with people who used the service and they told us that staff understood their needs and were supportive. They also felt that the house manager's were approachable. One person said, 'The staff are supportive and know what I need.'

However, we found that records did not always reflect the assessed needs of the people who used the service.

We found that staff felt supported by their managers and they could request a supervision session if they needed to discuss anything.

During an inspection in response to concerns

We carried out this inspection because we received some information of concern regarding staffing shortages. We spoke to people who used the service but their feedback did not link to this inspection. This was because we wanted to look specifically at how staff were deployed and supporting information.

We found that there were staff shortages which were not being adequately dealt with by the provider. The arrangements for covering shifts were not adequate.

Many of the people who used the service could not tell us directly about their care due to a variety of complex needs. However, we spoke to three people who used the service and they told us that staff were very supportive and that they liked living there.

During an inspection looking at part of the service

We spoke with several people who used the service about the cleanliness of their room and of the home. A person told us 'I think the place is clean.' The person helped to clean their own bedroom. Another person told us they cleaned their room by themselves.

We spoke with a relative who told us that they noticed that previously the home was not very clean, but they went on to tell us, 'The last few times that I've visited it's been perfectly OK. My relative's bedroom looked perfectly acceptable to me. There were no offensive smells, and never have been. Other areas are perfectly acceptable. The staff make a really good effort in doing it all.'

During an inspection in response to concerns

Many of the people who use this service could not tell us directly about their care due to a variety of complex needs. However, we visited all four houses and observed that people were treated with dignity and respect.

We saw that people using the service were offered choices and supported in their daily living. People we spoke to responded positively about their experience of care and support offered to them.
