Effective -This key question has been rated Good. This meant people's outcomes were consistently good, and people's feedback confirmed this. People's needs were assessed and care and support were delivered in line with current standards to achieve effective outcomes. People had healthcare plans in place which detailed their health needs and the support they required. The provider ensured the service worked effectively within and across organisations to deliver effective care, support and treatment. Mental capacity assessments and best interest decisions were detailed and had been completed in line with best practice. People were supported to make choices and staff were skilled in identifying the needs and preferences of people who were unable to verbally communicate.

Assessing needs

Records evidenced people’s needs were regularly reviewed through assessment to ensure their support plans continued to reflect their needs.

The management team told us about their assessment process and how individualised transitions plans were created for people joining the service, this included the use of social stories to aid people’s understanding of the process.

People’s needs had been assessed prior to moving into the service and involved the person, their relatives and external professionals. People had regular opportunities to contribute to the review and updating of their care plans.

People were supported to make choices and staff were skilled in identifying the needs and preferences of people who were unable to verbally communicate. We talked to people about getting up and going to bed and one person told us about how they liked a late night and said it was "my choice".

Staff told us about the training they had received on the mental capacity act and how they followed the mental capacity act in their day-to-day practice. One staff member told us, “People make choices, and we encourage this by using pictures and objects.”

The provider was working in line with the Mental Capacity Act. Mental capacity assessments and best interest decisions were detailed and had been completed in line with best practice. Where people were deprived of their liberty, DoLS were in place and people were supported in line with their agreed plans.
