During an inspection looking at part of the service

We saw that care plans and risk assessments had been regularly reviewed and updated and were signed and dated. Each document included a review sheet to plan and record regular risk assessment and care plan reviews. We also saw that documents such as cultural needs assessments, 'When I die' and Hospital Passports had also been reviewed and updated. We saw that during staff supervision key workers were required to demonstrate that all files were up to date and accurate. We also saw that monthly key worker meetings that included the person who used the service reviewed the individual's folder to ensure that all the information held was up to date and relevant to the person's needs and care.

During a routine inspection

As part of our inspection we spoke with relatives of people who used the service. Relatives told us that they had been involved in the care for their relative and the care plans had been reviewed regularly. One relative told us "There was a very good care plan review only in December. They were marvellous, very organised, had copies of all the relevant documents and discussed everything with me and how things would help. They really take notice of her needs and are very interested in changing things for her".

We reviewed that care plans for three people and saw that wherever possible consent was sought from the person who used the service. We also saw that relatives and other people involved in the care of the person were included in decisions relating to the individual's care where people had no or limited capacity to give consent.

We saw that the service had taken steps to provide care in an environment that is suitable designed and adequately maintained. One relative told us the environment "is absolutely splendid".

We observed that there were sufficient staff throughout our visit to assist people when they needed support. Staff had the appropriate skills, qualifications and experience to meet peoples' needs. One relative we spoke with said "All the staff are excellent".

However we found concerns that accurate and appropriate records had not been maintained and kept safe in order to protect people from the risks of unsafe or inappropriate care and treatment.

During a routine inspection

Some of the people in the home had complex needs which meant that they were not able to tell us their experiences of using the service; we therefore used our observations to help inform some of our judgements. We saw that people were being supported around the home by staff in a kind and sensitive manner, in a way that promoted individual independence. For example, we observed that people who required help to prepare their lunch time meal were given appropriate assistance by staff. We saw staff offering people a choice of drinks and supporting people to eat their food in a respectful manner that promoted individual independence wherever possible. We saw that staff recognised individual signs of anxiety or discomfort, and were able to respond effectively to this.

One person we spoke with indicated that they liked living in the home and the staff who cared for them. We arranged to speak with some relatives over the telephone. They told us that they were happy with the care and support provided by the service. Comments included 'The service is brilliant, very friendly, we would not want our relative to go anywhere else'. We found that relatives and people who used the service had been involved in their care.

We also gathered evidence of people's experiences of the service by reviewing feedback questionnaires completed in 2012 by professionals, people using the service and their representatives. We found that people had recorded that they were happy with the service.
