During an inspection looking at part of the service

When we inspected the service in September 2013 we found that people had not always been protected from the risks of receiving inappropriate or unsafe care.

At this visit we saw that information in care plans was now consistent and had been regularly reviewed and updated where necessary. We also found that assessments had been carried out when people had been identified as at particular risk.

People we spoke with were positive about their experiences of the service. One person said 'I absolutely love it here. I have no complaints whatsoever and I love the carers to bits'. We observed that staff treated people with respect and kindness.

Our inspection of September 2013 found that medicines had not been managed safely. At this visit, we saw improvement in some areas but we continued to have concerns about how people's medicines were administered and recorded.

In September 2013, we found that records about care had not been accurately maintained, placing people at risk of receiving inappropriate or unsafe care and treatment. During this visit we read five care files and reviewed other documents about the care delivered to people using the service. We found that, apart from records relating to medicines, there had been a marked improvement in record-keeping.

During an inspection looking at part of the service

During our inspection we spoke with four people who received care and three relatives. Some people told us that the care received was 'Excellent' while others felt that newer care staff were less well-equipped to meet people's differing needs.

We read seven care plans and found that, while some information was detailed, risk assessments had not been appropriately carried out in order to protect people's safety and welfare.

We checked the medication and systems at the service and found that staff had received the relevant training in medicines administration. However we saw that there were no lists of medicines on people's care files and that medication administration charts did not itemise the drugs to be given. We also noted gaps and inaccuracies on some charts and that special instructions for particular medicines had not been followed by the service.

We found that care; staff and complaints records had not been maintained to an appropriate standard. There were many contradictions in the documents held about people's care; some of which could have place people at risk.

During a routine inspection

Peoples care was planned and delivered in ways to address their individual needs. Care plans contained detailed information about peoples needs and preferences.Risk assessments had been made about different aspects of the care provided and peoples consent had been obtained.

We spoke to five people who told us that they were happy living in the service. One person said: "I can't faut the care, the staff are wonderful".

People were protected from the risk of abuse and they told us that they felt safe and had a relationship of trust with the care staff.

Medication was managed appropriately and peoples differing needs for support had been considered and recorded.

All staff had received up to date medication training.

The provider had an effective system to regularly assess and monitor the quality of service that people received. People had been invited to complete a survey in which to voice their views of the service. We read comments in the surveys which said: 'I'm very happy with the support so far' and 'I have no issues, the staff are all very good'.

Regular audits had been undertaken and improvements made as a result of the findings.

We found that some of the records at the service were not accurate and fit for purpose.

We reviewed four care plans and saw that there was no record of regular reviews. There was contradictory information about the number and times of peoples care visits which could have led to confusion and inappropriate care being given.

During an inspection looking at part of the service

We spoke to seven people who used the service. They told us that they were happy with the care and support they received. They said that the staff were very good and gave them the time they needed to perform certain tasks. One person said, "I've been here two years and the staff are nice. I never feel rushed", another said, "The staff are fantastic". We found that the care and support people received was delivered respectfully and in a way that allowed people to feel comfortable in their home. People were given the opportunity to be involved in their care and were asked for their consent where necessary. We saw that the service had put measures in place to ensure people's safety and people we spoke to felt safe in their surroundings.

During a routine inspection

People told us that they were given the care and support they needed, staff were kind and caring and care was delivered safely. They knew what care they would receive each day and knew the staff working with them. They liked receiving their support from an agency that had its office and staff based in their housing complex. Although they had set times for care they felt safe having staff on the premises for twenty four hours a day who could respond in an emergency, and they could have carers they knew and felt comfortable with if their needs increased.

They told us staff respected their privacy and dignity and were polite.

People had information on how to complain if they needed to and were consulted about their views on the service.
