People’s immediate and ongoing needs were fully assessed. Where appropriate this included their clinical needs and their mental and physical wellbeing. People were advised what to do if their condition got worse and where to seek further help and support. People were involved in the assessment of their needs, and support was provided where needed to maximise their involvement. The needs of carers of people using services were assessed and met.

Assessing needs

Leaders and staff told us the practice would use a series of codes and alerts on the patient record to highlight people’s communication needs and any impairments. The practice had systems and processes in place to identify people’s needs and preferences during the registration process. Staff told us they checked people’s health, care, wellbeing and communication needs during health reviews.

People told us they were involved in the assessment of their needs and support was provided to maximise their involvement. People told us they felt their individual needs were appropriately assessed and fully understood by practice staff.

The practice had a system in place to identify people with caring responsibilities. The carers lead held a carers clinic and drop-in service to support carers. The practice worked closely with social prescribers to assist people access additional support within the local community. The practice had a system in place to ensure people’s assessments were up-to-date and staff understood people’s current needs. People’s care needs were routinely reviewed. People’s needs were assessed using a range of assessment tools. People with long term conditions were invited for an annual health check.
