The systems and processes in place did not always ensure people’s records were up to date due to gaps in repositioning records. Care plans were detailed, and person centred. Staff were trained to meet people’s needs. People had access to healthcare services. Records showed health professionals were accessed when needed and the service took appropriate action.

Assessing needs

We only received limited feedback from people regarding this specific quality statement, although 1 person told us, “They’re attentive. I’ve got no concerns.” No concerns were raised by relatives with regards to how their family members assessed needs were managed.

Staff were knowledgeable about people who required assessment and ongoing reviews relating to skin care, the use of equipment and for those who were at risk of falls. We spoke with the registered manager about assessments, who told us, they carry out a preadmission assessment to find out people’s likes, dislikes, cultural differences, and cultural celebrations. They gave an example where the staff rota was adapted to support one person whose first language was not English. Then worked closely with the family to understand their cultural needs, including their food preferences.

Care plans provided information about how people wished to receive care and support. However, we found gaps in people's care records, this included gaps in people's repositioning charts, pressure mattress checks and up to date photos were not available in people's wound care plans. Clinical reviews took place regularly and provided assurance that people were receiving their care as planned including when they became unwell. The service used RESTORE 2 this is a physical deterioration and escalation tool used in some care and nursing homes. It is designed to support homes and health professionals to recognise when a person may be deteriorating or at risk of physical deterioration.

Supporting people to live healthier lives

Records showed health professionals were accessed when needed and the service took appropriate action to keep people healthy.

People told us staff supported them to access healthcare services. Records showed health professionals were accessed when needed and the service took appropriate action to keep people safe. These included, general practitioners (GP), speech and language therapists (SALT), and tissue viability nurses (TVN).

Staff were knowledgeable about people needs and knew how to support them to live healthier lives. Staff described the actions they would take in the event of a fall. We were satisfied with their responses. Nursing staff spoken with were confident on how to meet clinical needs for people using the service. The registered manager told us, “If someone has had a fall a nurse should see the person before they move and do an assessment. Neurological assessment, a post falls assessment and huddle with those involved. We will check for injuries or bruising. Obvious if there is an injury then we would contact 999. Review the care plans and put things in place to minimise it from happening again. I am confident my own nursing staff follow it the correct procedures.”
