People's care and support was co-ordinated. People, and their relatives, received accurate information about their support. People were encouraged and supported to be involved in planning their care and support. Relatives were involved, where appropriate.

Equity in experiences and outcomes

People told us they received equity in their care and outcomes, and received timely access to other professionals. One person explained, "They (staff) work with the chiropodist and the GP."

Care plans were person-centred for each aspect of people's lives and care. Care plans contained detailed information to ensure people received outcomes specific to their needs and wishes. The service tailored care delivery to meet the individual needs of the people living there.

Staff described how everyone was supported to participate in activities, even if they were in their rooms. For example, one staff member explained, "We tell [person] about everything that is going on. If we have activities in the sun room or we have a singer in we get the ipad and [person] can sit and watch on the i-pad in [their] room." Another staff member said, "We have someone on a special diet and we include [them] in meal times, everything is in [their] care plans about what their likes and dislikes are." Another staff member described how they support people, "(We let) people make their own choice, everyone has their own beliefs. Treating [people] as individuals, what they like and don't like and adapting to it."
