Description
An Improvement Notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 following a planned comprehensive inspection of the brachytherapy service. This identified concerns around the quality assurance processes for documentation, adequate authorisation guidelines and correct information held on the equipment inventory.
Result
The employer submitted evidence and was deemed to comply with this Notice on 24 October 2023.
