Breach
On 27 September 2022 The employer was found in breach of regulations 8(4)(a)(ii), 8(4)(a)(iii) and 8(4)(a)(iv).
Description
An improvement notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 following an inspection of the radiotherapy service. This identified concerns around accidental and unintended exposures. Incidents meeting the criteria of ‘significant accidental or unintended exposures’ as per professional body guidance, had not been reported to the CQC IR(ME)R team. We saw no evidence of written documentation of incident management including assessment against significant accidental and unintended exposures (SAUE) criteria.
Result
The employer submitted evidence and was deemed to comply with this Notice on 22 November 2022.
