Breach
On or before 12 July 2023, the employer was found in breach of regulation 8 Accidental and Unintended Exposure.
Description
An Improvement Notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 following a planned comprehensive inspection of the brachytherapy department. This inspection identified concerns around the management of clinically significant unintended or accidental exposures as there was no defined procedure for the assessment of incidents to see if they met the clinically significant unintended or accidental exposure threshold.
Result
The employer submitted evidence and was deemed to comply with this Notice on 23 August 2023.
