Breach
On and before 27 October 2022 the employer was found in breach of regulation 11(5).
Description
An improvement notice was issued to the employer under Section 21 of the Health and Safety at Work etc. Act 1974 following an inspection of the brachytherapy service. This identified concerns around the arrangements for delegated authorisation. We saw the department used a letter listing named unlicensed practitioners, signed by the licence-holding practitioner, to delegate authorisation of brachytherapy procedures, rather than authorisation guidelines as required by the regulations.
Result
The employer submitted evidence and was deemed to comply with this Notice on 21 December 2022.
