Characteristics of a service rated inadequate in this area

Management and staff do not understand the principles of good quality assurance and the service lacks drivers for improvement. Staff are not adequately supervised and staff turnover may be high. Staff may refer to people in an inappropriate way.

There is little or no evidence of learning, reflective practice and service improvement. Information to support performance monitoring and making decisions is inaccurate, invalid, unreliable, out of date, irrelevant or not gathered.
