Review of health and social care in Halton

Today we are publishing the first of our targeted reviews of how well older people can move through the health and social care system within a local authority area.

It focuses on how services, for example hospitals, GPs and care homes, work together.

The review found a strong commitment to serve people in Halton, Cheshire, well. This included helping to maintain people's health and wellbeing in their own homes and giving good support for carers. A rapid clinical assessment team and seven-day re-enablement service was helping to reduce delays in transfers of care.

This continuous improvement is crucial to meet the challenges of increased demand.
