Updated registration application form published
In April 2015, we introduced new application registration and variation forms for care providers.
The new forms were developed to take account of the changes to regulations from 1 April, including the introduction of the fundamental standards. They also reflect our new approach to regulation, which looks at whether care services are safe, effective, caring, responsive to people's needs, and well-led.
On 4 June, we introduced an updated version of the registration application form and published the form on our website.
This updated version of the form includes the following changes:
- Corrections to formatting errors.
- Additional requests for information regarding franchise arrangements for partnerships and individuals.
- Merging of employment history and previous history as a registered person.
- Requesting date of birth for additional partners.
All providers should now use the updated version of the registration application form, available from this page. We will continue to accept and process the previous version of the form until close of business 17 June 2015. From 18 June, any applications received on old versions of the forms will be automatically rejected.
