The Five year forward view: one year on

It's a year since NHS England published the Five year forward view, which sets out a vision for the future of the NHS, and describes a range of new ways of breaking down the traditional divide between primary care, community services and hospitals.

We're one of the six organisations that signed up to the document and we are playing an active role in delivering its objectives.

It sets out a shared challenge for all involved in health and care – to close three gaps in healthcare: the health and wellbeing gap, the care and quality gap, and the funding and efficiency gap.

The Five Year Forward View has been at the front of our thinking for both our business plan for 2015/16 and the development of the strategy for 2016 to 2021. Over the past year, a number of our projects have been contributing to the implementation of the Five Year Forward View, such as our work on integrated care and quality of care in a place.
