Hammersmith and Fulham practice rated Inadequate by CQC

The Medical Centre – Dr Kukar, in the London borough of Hammersmith and Fulham, has been rated Inadequate overall by the Care Quality Commission (CQC) and has been placed in special measures.

The practice has been rated Inadequate for being safe, effective, responsive and well-led. It was rated Requires Improvement for being caring, following the inspection in June 2019.

Inspectors found the practice did not have safe systems and processes to keep patients safe. There was not appropriate safeguarding training in place for all staff. There was not safe infection prevention and control practices in place.

There was limited monitoring of the outcomes of care and treatment. The practice was unable to show that staff had the skills, knowledge and experience to carry out their roles.

Patients consistently described difficulties with making appointments.

Leaders could not show that they had the capacity and skills to deliver high quality, sustainable care and the overall governance arrangements were ineffective.

Inspectors saw limited evidence of systems and processes for learning, continuous improvement and innovation.

Areas where the practice must now make improvements are:

  • Ensure that care and treatment is provided in a safe way.
  • Establish effective systems and processes to establish good governance in accordance with the fundamental standards of care.

Areas where the practice should improve include:

  • Improving the identification of carers to enable this group of people to access the care and support they need.
  • Ensuring that all staff have an understanding of Duty of Candour.

Head of Inspection for Primary Medical Services in London, Antony Hall, said: “We have placed The Medical Centre – Dr Kukar into special measures. This should give people who use the service the reassurance that the care they get should improve.

“We will inspect again within six months and if there has not been the required improvements, CQC could escalate to urgent enforcement action.”
