Westfield Surgery is situated in a residential area in the Herefordshire market town of Leominster. It has 9,180 patients. The practice is in a purpose-built building which is shared with another GP practice. The practice population is the fourth least deprived decile in England. Level one represents the highest levels of deprivation and level ten the lowest. The practice has a General Medical Services contract with NHS England.

The practice has two GP partners, two salaried GPs (both male and female). The practice has two advanced nurse practitioners, three practice nurses, two health care assistants and a part time pharmacist. The clinical team are supported by a practice manager and a team of administrative and reception staff.

The practice treats patients of all ages and provides a range of medical services including NHS health checks, family planning, well-woman, baby clinic, minor surgery and nursing services. It also provides care for two local nursing homes.

Parking is available on site and the practice has facilities for disabled patients.

A chaperone service is available for patients who request the service. This is advertised throughout the practice.

The practice is open between 8am and 6pm Monday to Friday. Home visits are available for patients who are too ill to attend the practice for appointments.

The practice does not provide an out of hours service to their own patients. When the practice is closed patients are directed to contact Primecare via 111.

We carried out an announced comprehensive inspection at Westfield Surgery on 25 October 2018 as part of our inspection programme.

At this inspection we found:

  • The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
  • The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
  • The practice had completed an infection control audit and action had been taken to address areas identified. However, the practice needed to embed this further to ensure it was reviewed and monitored regularly in line with its policy.
  • The practice involved patients in regular reviews of their medicines.
  • The practice understood the needs of its population and tailored services in response to those needs. For example, providing home visits for patients with long-term conditions.
  • There was evidence of a number of quality and pilot initiatives to ensure patients’ needs were met.
  • The practice engaged with external organisations to improve the quality of services delivered for practice patients and the local population.
  • Staff involved and treated patients with compassion, kindness, dignity and respect.
  • There was a strong focus on continuous learning and improvement at all levels of the organisation.

The areas where the provider should make improvements are:

  • Further develop an effective system to review and monitor infection control.
  • Ensure a stock of appropriate emergency medicines are available to cover potential emergency situations.
  • Continue to monitor and review the uptake for cervical screening.
  • Continue to monitor patient satisfaction rates particularly in relation to access to services.
  • Continue to actively encourage patients to join the patient participation group.
