During an inspection looking at part of the service

This was a follow up inspection to our visit on 22 January 2014. During that inspection we found short falls in the process used to ensure that people's medication records were accurate following care from other healthcare professionals. During this visit on 28 July 2014 we found the provider had put in place a safe system to make sure that changes to people's medicines were timely and accurate. The provider had also put in place an appropriate system to monitor this process.

During a routine inspection

We visited the practice on 22nd January 2014 to look at medicines management, we did speak with staff but on this occasion we did not speak with people using the service. We found that medicines were available, stored appropriately and fit for use. However, people were not protected against the risks associated with medicines because the practice did not have an effective system to ensure that medication changes that occurred in hospital were acted upon. The practice could not ensure that their current medication records were up to date.

During an inspection looking at part of the service

During this inspection we followed up on concerns we identified at our previous inspection in June 2013. We did not speak to patients during this inspection.

We found appropriate hygiene and infection control guidance was being followed. The practice undertook checks on risks related to healthcare associated infection.

There were systems to identify, investigate and learn from significant events. Concerns identified during our last inspection were investigated and appropriate action was taken.

During a routine inspection

Patient care and treatment was planned to reflect patient cultural needs, preferences and diversity. We found the practice had a range of clinics to support the patient population. One patient told us 'The staff are passionate and experienced. They treat me with respect and are always very professional'.

We found that patients were protected from the risk of abuse. This was because staff knew about the types of abuse and who to report any concerns to. Patients we spoke with told us that they felt safe with the GPs and practice nurse.

The practice was clean and tidy. However, we found the practice did not follow appropriate guidance for infection control.

Staff received appropriate personal development. The practice held annual appraisals and regular team meetings.

The practice did not have and effective system to ensure significant events were identified and investigated.

Patients who used the service, their representatives and staff were asked for their views and feedback. We saw the results of a recent survey. Most of the patients who responded to the survey stated the service was good.
