People should get safe and coordinated care when they move between different services (outcome 6)

Meeting this standard

We checked that people who use this service

  • Receive safe and coordinated care, treatment and support where more than one provider is involved, or they are moved between services.

How this check was done

We looked at the personal care or treatment records of people who use the service, carried out a visit on 28 June 2013, observed how people were being cared for and checked how people were cared for at each stage of their treatment and care. We talked with people who use the service, talked with carers and / or family members, talked with staff and talked with commissioners of services.

Our judgement

People’s health, safety and welfare was protected when more than one provider was involved in their care and treatment, or when they moved between different services. This was because the provider worked in co-operation with others.

Reasons for our judgement

People we spoke with told us they were registered with a local doctors’ surgery and said the staff at Cartmel Old Grammar “always” contacted their doctor if they felt unwell.

The records the home held about people showed they were supported by a range of health care services. We saw that people who had more complex care needs had access to appropriate specialist services as they required.

The records we looked at showed that staff in the home had contacted health care services for support and advice as people needed this. We saw that where advice had been provided by a health care service, individuals’ care plans had been updated to include the advice given. This ensured people received appropriate care and treatment to meet their needs.
