During a routine inspection

Street Med provide an outreach and case management service to ensure that no rough sleeper or resident of homelessness services fails to have access to healthcare provision. We visited the service's office but did not have the chance to speak with people who use the service due to its nature.

The service supported around twenty people with complex needs at the time of our inspection. We found that their needs were assessed after appropriate consent was gained and action plans were created in order to enable and support them to meet their needs, access other services or be safeguarded against harm.

Staff we met were enthusiastic about their service and the support they provided. We spoke with two nurses who gave us feedback on their experiences working for the service. They said 'It's good' and they enjoyed seeing 'making a difference' to people's lives. Other comments included 'Really like it' and 'It's more challenging than ordinary nursing', which was a positive thing about the job for this member of staff.

During a routine inspection

We spoke with an organisation that works in partnership with Street Med. The organisation told us that Street Med had been instrumental with achieving positive outcomes for people using their service. We were given examples of how chronically unwell homeless people had been supported to access health services.

The staff told us that they worked intensively with people who had been homeless for years. This involved a detailed assessment process and referring people on to appropriate services. This enabled staff to work with service users effectively meeting their identified complex health and social care needs.

We saw evidence and details of the guidelines and policies that were based on best practice and used by the team. This included the findings and guidelines from a national homeless project, called Open doors, and the paper No One Left Out - Communities Ending Rough Sleeping (2008).
