During a check to make sure that the improvements required had been made

The provider sent us evidence to show that they had made improvements in order to achieve and maintain compliance with outcomes 14 and 16. We found that they had completed most of their planned actions and had begun to measure the impact of these improved systems and processes.

Staff were better supported in their roles because they had access to clinical supervision and their attendance at mandatory training was being monitored. This enabled them to reflect on their work and develop their knowledge and skills to meet the needs of people using the service.

The arrangements for monitoring the quality and safety of the service had been improved, which enabled the provider to make continuous improvements to the way the service was delivered.

During a routine inspection

We carried out this inspection in partnership with Her Majesty's Inspectorate of Prisons (HMIP). We spoke with prisoners on an individual basis to ask for their opinion on the healthcare service provided. We spoke with members of the staff team and the registered manager. We looked at a range of records and observed staff interactions with prisoners. HMIP completed a pre-inspection questionnaire and this asked specific questions about the healthcare services.

The majority of prisoners we spoke with were satisfied with the service received from the provider. Comments included 'they support me as best they can'. One prisoner told us that a member of Derbyshire Health United (DHU) staff had been 'sympathetic and treated me with respect'. Most of the prisoners we spoke with told us that they had been involved in their care and treatment.

We found that the directly provided services operated to a good standard and their working relationships with other healthcare organisations led to effective care being delivered.

We found that the providers systems for assessing and monitoring the quality of the service were not adequate.

Staff we spoke with had access to informal supervision through peers and line management within their team. However we found that there were no formal structure in place to ensure staff had access to regular supervision and appraisal.
