During a routine inspection

We inspected Summerfield House Nursing Home on 30 July 2015 and the visit was unannounced.

Our last inspection took place on 10 April 2013 and, at that time, we found the regulations we looked at were being met.

Summerfield House Nursing Home is a 106-bed purpose built service and is registered to provide accommodation and personal care for older people and people living with dementia. Nursing care is provided. At the time of our visit there were 99 people using the service.

Summerfield House Nursing Home is a is a purpose built care home with nursing situated approximately 1.5 miles from Halifax town centre. The accommodation is arranged over three floors. All of the bedrooms are singles with an en-suite toilet, some also have a shower. There are lounges and dining areas on each floor.

The service has a registered manager. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

Although people told us they felt safe we found we found when people had reported missing property staff had not followed the procedure to ensure a thorough investigation.

Recruitment processes were not robust as thorough checks were not always completed before staff started work to make sure they were safe and suitable to work in the care sector.

There were enough staff on duty to make sure people’s care needs were met and activities were on offer to keep people occupied and stimulated. We saw people enjoying a book club meeting and music for heath session during our visit.

Staff told us they felt supported by the manager and that training opportunities were good. People and relatives we spoke with told us they liked the staff

The home is well appointed, well maintained and comfortably furnished. People’s bedrooms were personalised and we found everywhere was clean and tidy.

We found people had access to healthcare services and these were accessed in a timely way to make sure people’s health care needs were met. The medication system was well managed and people received their medicines at the right times.

On the day of our visit we saw people looked well cared for. We saw staff speaking calmly and respectfully to people who used the service. Staff demonstrated they knew people’s individual preferences and what they needed to do to meet people’s care needs.

We found the service was meeting the legal requirements relating to Deprivation of Liberty Safeguards (DoLS).

People told us the meals were good. There was a choice available for each meal and the chef was aware of people’s preferences.

Visitors told us they were always made to feel welcome and if they had any concerns or complaints they would feel able to take these up with the manager.

We saw there were systems in place to monitor the quality of the service. When areas for improvement were identified action was taken to address the shortfalls. People using the service were asked for their views and the registered manager was in the process of responding to their requests.

During a routine inspection

We spoke with ten people who live at the home, five relatives, three visiting professionals and nine members of staff. These are some of the things they told us:

'It's first class here. The staff are very good and whatever I want I can get.'

'We've got a good activities person, we made Easter bonnets which was a lot of fun. We do other things like baking, art activities and games. We also sit as a small group and chat which is very nice.'

'On the whole the food is very good.'

'It's lovely here and the staff are very kind.'

'The staff are friendly and always smiling. I am always made to feel welcome when I visit and it's like a first class hotel. My relatives' wellbeing has improved since they moved here.'

'This was the first nursing home we looked at and we made the right choice.'

'My relatives' health has improved since they moved here and they are joining in with the activities. The staff are very good and you only have to mention if something needs doing and it's done.'

'The accommodation is very good. The home is always clean and there are no unpleasant odours or my nose would be twitching.'

'The staff group are very friendly. I haven't been here long and I'm really enjoying working here.'

'It's a good home, one of the best I visit.'

During an inspection in response to concerns

We spoke with 10 people living in the home and three relatives. These are some of the things they told us:

'The staff are very nice and helpful. The hairdresser comes twice a week and is very good.'

'The staff are very good, the night staff do very well with us.'

'We can get up and go to bed when we want to. Sometimes I stay up late but sometimes I like to go to bed earlier.'

'The meals are quite satisfactory.'

'It's very nice here the food is very, very good. The lunch today was beautiful, it was quiche.'

'The home is very clean and I like my bedroom.'

'I am very satisfied with the care and support my relative receives.'

During an inspection looking at part of the service

We spoke with five people who use the service, one relative and six members of staff. These are some of the things they told us.

'There are more staff on duty now and this has made things better.'

'One of the new nurses is very good I would tell them if I wanted something and they would sort it out.'

'I have noticed that there are more staff around and things seem more organised.'

'Meal times have improved and are more organised.'

'It is much nicer working here now each carer has four people to look after on each shift. This is working well there is a better routine and work is much more enjoyable.'

During an inspection looking at part of the service

We spoke with ten people who live at the home and one relative. These are some of the things they told us:

'I like it here everyone is friendly.'

'I don't think there are enough staff. Sometimes I have to wait for the carers.'

'I like my bedroom.'

'I like the entertainment that comes in once a month.'

'Sometimes I have to wait up to ten minutes for staff to take me to the toilet.'

'Staff do their best, but we have to wait because there aren't enough of them.'

'Staff are kind, but there is not much going on. We spend a lot of time just sitting.'

'Food is good as far as they can do it.'

'I'd like to be in my bedroom listening to the radio but I'm not allowed because I've had a few falls. I can't see very well and like to listen to the cricket or rugby.'

'I am happy with the care and support my relative gets.'

'One staff member on nights and some others treat me like I'm a rag doll.' They don't try to make me comfortable. They refuse to take me to the toilet at night after I've rung so many times. They say you have to wait, I have to p*** the bed. I have trouble speaking and they leave before I can tell them what I want. Another member of staff is rough, they pulled my call bell out of the wall.'

During a routine inspection

We spoke with six people living in the home and three relatives. Everyone told us that they liked the accommodation at the home and that the home is kept clean and tidy. All of the relatives told us that they are made to feel welcome and can make themselves a drink and stay for a meal if they want to.

People's views about the staff were mixed, people said 'Some staff are better than others, most are very caring,' 'the staff are all nice' and 'the staff are good.' People told us that staff treated them with respect and records showed that staff are getting training and support.

From talking to people and looking at the care records it showed us that people and /or their representatives were not being involved in planning their care and support. Because of this care plans lacked detail and were not personalised to show how staff should meet individuals' needs.
