During a routine inspection

Some of the people who use the service had complex needs which meant that they were not able to tell us about their experiences of using the service; we therefore used our observations to help inform some of our judgements. We observed staff interacting with people who use the service in a respectful manner, calling people by their first names and offering choices.

People who use the service appeared relaxed, content and at ease in their surroundings.

We spoke with three members of staff and the registered manager during our inspection. One staff member told us, "This home is definitely service user led".

Records showed that people who use the service were encouraged to be active and be part of the local community. People were able to engage in a wide range of activities.

Staff were very supportive to people and there were arrangements in place to make sure the risk of confrontation between people using the service was minimised and that they were kept safe.

We found appropriate arrangements were in place in relation to the obtaining, storage and dispensing of medicines.

The home was very well managed and there were clear lines of leadership in place, meaning the home was organised and communication was effective.

The provider had quality assurance systems in place.

During a routine inspection

We made an unannounced visit to the service and spoke to people who use the service, the acting manager, the quality assurance manager and to staff members. There were 10 people using the service. We met and spoke with some of them and everyone we spoke with said or indicated that they were happy living at Twyford House.

People told us or indicated that they felt safe and well looked after.

People indicated that they could express any problems to the staff who would listen and act. People indicated that they thought the staff were kind and caring. Staffing was planned around individual needs and activities.

People looked happy and relaxed in the company of each other and staff. We saw that people took part in a range of community based activities that they enjoyed. People went to and returned from various clubs and activities during our visit.

People expressed that the home was clean and that their bedrooms were kept clean. People had support to take part in the cooking and cleaning and had the opportunity to go food shopping and prepare their own meals.

People's health needs were supported and the service worked closely with health and social care professionals to maintain and improve people's health and well being.

During an inspection in response to concerns

We made an unannounced visit to the service and spoke to people who use the service and to staff members. There were 10 people using the service. Due to some concerns raised by a recent incident at the service, we looked at the way the service supports problem behaviour and uses physical intervention and restraint.

People said or expressed that they were happy living at Twyford House.

People said that they were happy with the rooms and facilities. One person said they thought their bedroom was 'brilliant'.

People who use the service told us or expressed that they felt safe. We saw that people were relaxed in the company of each other and staff.
