During a routine inspection

We found that the registered manager had a robust cleanliness and infection control policy in place. This was supported by comprehensive cleaning schedules and procedures as well as the maintenance of policies relating to the use of protective equipment. People we spoke with told us that the service was clean. One person said "They have recently got a new housekeeper and they are really on top of it. Full marks to them."

We saw, during the inspection of the premises that all areas of the building were clean and hygienic.

We looked at records relating to the monitoring of the service and these included a wide variety of audits covering all aspects of service provision. The registered manager had ensured that the opinion of people using the service, or their relatives or representative, had been sought and acted upon.

One relative with whom we spoke told us "The staff are brilliant, my relative has only got to sneeze and I know about it."

During a routine inspection

The majority of people living at Melbreck were unable to respond to our direct questions as a result of their disabilities. We used a number of other methods to gain an insight into their experiences. This included observations of staff interactions with people, discussion with staff and relatives, and a review of documentary information.

People were happy and comfortable in the environment. Relatives said "The care the residents get is very good." Staff were said to be respectful and treated people with dignity and compassion. Residents enjoyed the activities available, such as physiotherapy and foot massages. We saw too that food was enjoyed and that choices were provided. Relatives commented that the "Standards were excellent." Staff ensured that appropriate support was given to enable people to eat and drink.

People had been encouraged to make their bedrooms their own through decoration and furnishings of their liking. Visiting relatives said they felt involved in the decision making processes where it concerned the care and well-being of their family member.

Relatives said that their family members rooms were kept clean. There were some arrangements in place to ensure infection prevention and control measures were taken but that these would benefit from some additional attention in order to support staff.

The service had appropriate systems in place for staff recruitment. Improvements were needed for monitoring the quality of service.

This review was carried out using documentary evidence provided by the service. We did not visit Melbreck to carry out this review of improvements made and we did not, therefore, speak to persons who use the service or their relatives or representatives.

During a routine inspection

We used a number of different methods to help us understand the experiences of people using the services these included spending time with people and talking with them. During some of the time we used a specific way of observing care, referred to as the Short Observational Framework for Inspection, (SOFI) to help us understand the experience of people who could not talk with us due to the complexity of their needs.

We also gathered information by talking with some staff on duty and people's relatives on the telephone following the inspection. We also looked through some of the completed quality assurance questionnaires that had been recently returned to the service from staff and people's relatives and some responses have been included within the report.

During an inspection looking at part of the service

Due to people's special communication needs it was difficult to ask people directly about the outcomes within this compliance review.

We spent time with people and observed their interactions with each other, their environment and staff.

It is acknowledged that the service has had a significant time of upheaval due to the lack of effective management on a day to day basis. This has resulted in the service having to completely review many of the fundamental systems which have been institutional and not person centred.

The providers have ensured since that last inspection in September 2011 that a robust management team have been working at the service in order to bring the service to a level of compliance.

The commission had received some concerns regarding the level of care and support provided by the home. The concerns had been referred to the local authority safeguarding team prior to our visit.

During this inspection we also included some elements of the safeguarding concerns we received and focussed on the compliance actions set during our last visit to the service on the 22nd September 2011.
