During an inspection looking at part of the service

At the previous inspection of this service in April 2013 we found that improvements were needed with regard to the administration of medication, staff support and supervision and quality assurance processes. At this inspection we found that improvements had been made, but that there were still issues of concern relating to medication. We found that staff received support and the opportunity for professional development. Staff had regular one to one supervision with their manager and undertook various training courses. These included training about safeguarding vulnerable adults, health and safety and autism. We found that the service had systems in place for monitoring the quality of care and support provided. These included systems for seeking the views people who used the service such as service user satisfaction surveys.

We found that medications were stored securely. However, we found instances of poor record keeping with regards to medication and one person was not being administered all the medications they had been prescribed.

During a routine inspection

People told us that they were involved with planning their care, and that they were treated with respect. One person replied "I choose myself" when asked if they could get up and go to bed when they wanted. People told us they were satisfied with the care provided. One person indicated by facial gestures and blinking that they were happy with the support provided. We found that care plans and risk assessments were in place. However, some risk assessments had not been subject to regular review. We found that medications were stored securely, but that accurate records were not kept n relation to medications.

The service was staffed in sufficient numbers to meet people's needs. We found that staff had received training which included safeguarding vulnerable adults and moving and handling. However, we found that staff had only limited one to one supervision with their manager. Quality assurance systems were in place, but these were not sufficiently robust to monitor and assess the quality of care on an on-going basis.

The service is registered for the provision of personal care. We found evidence to suggest that the service provides both accommodation and personal care and have informed the provider they are not registered to provide accommodation.

During a routine inspection

People told us they were treated with dignity and respect by staff. We observed that staff interacted with people in a polite and friendly manner. We saw that care plans were in place, and that risk assessments had been developed where there was an assessed risk. People told us that staff knew their care needs and how to meet them. One person said "yes, it's OK" when asked if their care needs were being met. Records showed that people had access to health care professionals.

Medications were stored securely, and records showed they were administered in line with how they were prescribed. The service had a complaints procedure, although this was in a format that not everyone living at the service could understand. We saw that complaints were investigated by the provider. We found that there were not enough staff to meet people's needs. One person told us "I want to chat with staff, but they never have the time." All of the staff (including the manager) told us there were not enough staff to meet people's needs. They also said that due to the high number of agency staff used, they often had to work with staff who did not know the people or how to meet their needs.
