During a routine inspection

We spoke with one relative, three people who used the service and did observations in communal areas during both days of this inspection.

One person told us, 'I can get up and go to bed when I want, I make my own decisions,' but we found mixed evidence in terms of whether people were treated with dignity, respect and were involved in decisions about their care. We saw some staff did not use respectful language when they referred to people living at the service.

There were significant concerns with the way nursing tasks were managed, especially around pressure area care. A visiting professional told us that some people who were at risk of developing pressure ulcers were not using the right equipment. In some cases they had the equipment they needed but staff were not using it correctly. A number of issues had been raised about pressure ulcers and some of the staff felt these could have been avoided, but said the nurses ignored their comments when they raised them.

We found that people did not always receive the support they needed to make sure that they received an appropriate diet to maintain their health.

People told us they felt safe at the home and we found that all incidents of potential harm were referred to safeguarding but not enough was done to prevent the risk of incidents.

We found there were not enough staff and the systems to support staff were not always effective.

Complaints were recorded and responded to appropriately.

During a routine inspection

Our observations of care delivery gave us mixed evidence. We saw some kind, gentle and respectful interactions between staff and people living at the service. Some staff had a genuine rapport with people living at the service and took time to converse with them, and to explain what they were doing. We saw that people living at the service seemed to appreciate this. We also saw lots of examples of people being offered choices, of what they ate, drank, did and watched on TV. People we spoke with told us the staff respected their decisions. We saw some staff talk about people's needs more loudly than was necessary in communal areas. In some cases staff were genuinely trying to establish how best to help people, but this could have been done more discreetly and respectfully.

People we spoke with told us they received the care they needed most of the time.

We observed that staff moved and handled people sensitively and safely, explaining and reassuring people whilst undertaking manoeuvres. The people we spoke with who had assistance to move told us they felt safe with the staff. They told us they had not seen or experienced staff behaving inappropriately towards people.

People told us they were happy with the changes they had seen since the new provider took over. They said they were very happy with the improvements being made to the accommodation and the garden. They said they were excited about the improvements being made to the activities and in particular were pleased the acting manger had asked them about where they would like to go on trips. They said the new providers were doing what they had promised them when they took over.
