During a routine inspection

At the time of this report the Millfield Nursing and Residential Care Home did not have a registered manager in post. The previous manager left the home without registering with the Care Quality Commission (CQC) and a new manager has been appointed. We were told they had just completed their four week induction period.

There were 33 people living at the home at the time of this inspection. We spoke with two people using the service and four members of staff and the newly appointed manager.

People told us the food was good and they were happy at Millfield . One person told us 'I am well looked after here,' and, 'They know me very well.'

People told us they had no concerns about the support they received and made positive comments about the care staff. We saw staff respected people's wishes to remain in their bedrooms or to use the lounge areas.

We saw people's care needs had been assessed, although not all of their care plans had up to date assessments about any risks that could affect them.

We saw that appropriate checks were undertaken before staff started working at the home, that helped to ensure that suitable people were employed.

We found that staff had not received recent refresher training that was important to ensure they could safely meet peoples needs.

People told us they knew how to raise any concerns or complaints and that they felt able to talk with care staff or their relatives if they did not feel safe.

During an inspection looking at part of the service

There were 33 people living in the home at the time of this inspection visit. We spoke with five people using the service, three visitors, three members of staff and the acting manager.

We found that the provider had taken action since our previous inspection to make changes and improvements to the service provided.

People using the service and their relatives we spoke with told us they were satisfied with the care provided. They said, 'I've no complaints about how I'm treated here', and, 'They know her very well, she gets all the care she needs'.

We found that care and treatment was usually planned and delivered to meet people's individual needs and to ensure their safety and welfare. We saw that care plans were mostly accurate and up to date.

We found that there were appropriate arrangements in place for the safe management of medicines in the home.

We found that people using the service, their representatives and staff were asked for their views about their care and they were acted on. Surveys had been used to gather views of the service provided. We saw that some action had been taken in response to comments made.

We found that records were mostly kept securely and could be located promptly when needed. We saw that there was suitable provision to ensure records were kept securely. However, we found that an office was left open and unattended during our visit which could have compromised the confidentiality of people's personal information.

During a routine inspection

In this report the name of a registered manager appears who was not in post and not managing the regulatory activities at this location at the time of the inspection. Their name appears because they were still a Registered Manager on our register at the time.

People told us they had no concerns about the care and support they received and made positive comments relating to the staff. We saw staff respected people's wishes to remain in their bedrooms or to use the lounge areas. We saw people were engaged in activities such as quizzes.

We asked people if they knew how to raise any concerns. They told us they felt able to discuss issues with any member of staff or the manager, and were confident the issues would be resolved.

We saw that people’s care needs had been assessed, although not all care plans were up to date. We saw that people and their relatives had been included in discussing their care and were able to make informed decisions about their care and treatment.

We saw that although systems were in place regarding medication, medication was not always administered safely.

We saw there had been recent investment in the building and a number of bathrooms had been refurbished.

We saw that the appropriate checks were undertaken before staff start work, to ensure that suitable staff were employed.

We found that systems were in place to quality assure the service provided but these were not being used to their full potential.
