During a routine inspection

We visited eight locations across the City of Bristol to meet with staff from Bristol Community Health. We spoke with teams and individual members of staff. These included community nurses and nurse specialists. In addition we visited South Plaza, the provider address to review their governance arrangements and recruitment information for staff. We also spoke with staff from the continuing health care team who assess people for nursing care funding.

We conducted telephone interviews with 12 people who used the service. During our visits we spoke with a further 14 people who used the service, 12 relatives and 26 staff.

Care was planned and delivered effectively to people by competent staff. People who used the service were given appropriate information and support regarding their care or treatment.

Robust recruitment systems were in place to ensure that staff were suitable to work for the organisation and vulnerable people.

There were effective systems in place to identify, assess and manage risks to the health, safety and welfare of people who use the service and others.

People were protected from the risks of unsafe or inappropriate care and treatment because accurate and appropriate records were being maintained.

During a routine inspection

We visited eight locations across the City of Bristol to meet with staff from Bristol Community Health. We spoke with teams and individual members of staff. These included community nurses, nurse specialists and allied health care professionals.

The locations included the Musculoskeletal Assessment and Treatment Service, The Haven, Charlotte Keel Health Clinic, Fishponds Health Clinic, the rapid response team based in the south of the city and the palliative home care support team based at Avonmouth.

We conducted telephone interviews with nine people who used the service. During our visits we spoke with six people who used the service.

Bristol Community Health provides care commissioned by the NHS to people. This was usually in the person's own home or at the various clinics they operate.

Care was planned and delivered effectively to individuals by competent staff. However, some improvements were required in relation to the written care plan. Three out of the fifteen plans we saw lacked necessary detail to ensure that consistent care and treatment was delivered and effectiveness could be measured. This was because statements in these three care plans were broad, for example, support with 'personal care' or 'support with prostate cancer'. It was not clear how or what support should be given to meet people's specific and individual needs.

People who used the service were given appropriate information and support regarding their care or treatment.
