During a routine inspection

The Sedgemoor Centre provided a range of community based services to people in their own homes as well as health clinics and GP surgeries. Services inspected included district nursing, community rehabilitation, end of life care and acute care at home services.

Surveys were used to obtain patients' views. We spoke with 11 staff with various community roles, for example, district nurses. Patients, relatives and carers commented positively about the services that supported them. We saw evidence that issues relevant to people who used the service were discussed and action taken.

Patients told us 'I only have to pick up a phone if I have a problem; whatever we have needed they have been able to cope with it very efficiently; the support we have had is second to none.' Survey responses about the district nursing service included: 'I have been receiving the highest level of care over the last few weeks from the nurse team. They are kind, caring and helpful and always make me feel comfortable and at ease.'

Care plans and visit records seen showed the involvement of the individual in their care and also that consent was sought.

Training was provided and staff were supported with team meetings and individual one-to one meetings.

The provider used a range of tools to monitor the effectiveness of the service provided.

During a routine inspection

Surveys were used to obtain patients' views. Patients, relatives and carers commented positively about the services that supported them. We saw evidence that issues relevant to people who used the service were discussed and action taken. For patients who used the telehealth service, a patient user forum met quarterly in three areas of Cornwall. Patient user forums were surveyed and we saw evidence that 50% of these responded.

Patients told us how they benefitted from the care and treatment provided. One relative told us, 'I only have to pick up a phone if I have a problem; whatever we have needed they have been able to cope with it very efficiently; the support we have had is second to none.' Survey responses about the district nursing service included: 'I have been receiving the highest level of care over the last few weeks from the nurse team. They are kind, caring and helpful and always make me feel comfortable and at ease.'

Patients told us how they appreciated the staff that worked with them. One relative told us, 'The nursing staff are very diligent; incredibly good.' Recent survey responses for the district nursing service included: 'They are a brilliant team doing a wonderful job,' and 'All your district nurses are first class, ten out of ten.' Responses to surveys of patients using the telehealth service included, 'Your telestaff are most sympathetic to any queries when I need advice.' This was typical of other responses. Patients felt safe in the service.
