During an inspection looking at part of the service

We carried out this unannounced inspection to follow up on compliance actions made at our inspection in January 2013. These related to staff recruitment and the quality assurance systems of the service.

During our visit to the office we reviewed four care plans and three staff files. We spoke on the telephone with three of the four people whose care plans we looked at.

People's needs were assessed and care was planned and delivered in accordance with their plan of care. People told us that staff were "very good" and supported people as described in their care plan.

Medications were handled, administered and recorded appropriately. The manager undertook an audit monthly and any gaps in records were investigated and explained. Staff had received training on safe administration of medicines and how to ensure records were accurately maintained.

Staff files showed that there were effective recruitment procedures in place and appropriate checks had been carried out prior to care workers commencing employment.

The provider had suitable systems in place to monitor the quality of the service provision. This included listening to and acting upon comments made and audits of care plans, to ensure information was accurate.

People we spoke with considered the service was well managed and they were able to approach the manager if they had any issues.

One person said they would give the service "a high nine out of ten, otherwise there was no room for improvement!"

During an inspection looking at part of the service

The manager of Prospect Tree Health Care is not registered with the Care Quality Commission to manage a regulated service.

In this report the name of a registered manager appears who was not managing the regulatory activities at the time of the inspection.

This was an unannounced inspection to follow up on a compliance action made at the last inspection relating to records not being completed accurately.

We visited the agency on 9 January 2013. The manager arrived during this inspection and asked us to leave the office and stop the inspection.

We addressed this with the provider and outlined CQC’s powers to enter and inspect regulated premises, as set out in the Health and Social Care Act 2008. We visited again on 17 and 21 January 2013.

We spoke to the manager, deputy manager and a senior care worker. We spoke with two people who received a service, one relative and a health care professional.

People were satisfied with the support they received from the agency. The relative told us “they are excellent quite honestly” and “there is nobody who comes that I can complain about”.

The provider had failed to ensure people were cared for by suitable staff because their recruitment methods were not robust.

The provider did not have an effective system in place to assess and manage risks to the safety and welfare of people using the service.

People’s personal records were accurate and fit for purpose.

During an inspection looking at part of the service

We carried out this inspection between 17 to 19 July 2012, to follow up on compliance actions made at the last inspection of the domiciliary agency in September 2011.

At the time of our inspection, a new manager was overseeing Prospect Tree Health Care. We were told that this manager, who was previously the deputy manager, was 'acting up' until January 2013 when a decision about a permanent management role would be made.

The manager told us that Prospect Tree Health Care provided a service to a small number of clients and that the Local Authority was working with Prospect Tree Health Care to improve the standard of service people received.

During the inspection we visited four people in their own homes and talked to them about their experiences of the care provided by Prospect Tree Health Care.

People told us that care workers treated them with respect, and provided all the help and care they needed. A relative we spoke with told us 'Carers are absolutely marvellous, very supportive to us as well as Mum'. Everyone we spoke with told us that they received their support from a regular team of care workers. They told us that the support and help they received was discussed and agreed with them. Individuals also told us that they thought the care workers had the competence to meet their needs. One person said, "what they do enables me to have a better life and do the things I want to do'.

We spoke with six members of staff and they told us they felt they had the right skills to undertake their role. One care worker said 'To promote independence, I try to get clients to do as much as possible for themselves'. Another care worker commented 'if we can help people in any way we will'.

All the care workers we spoke with felt supported by the manager and commented that recent training opportunities had supported them to care for people in the right way.

Individuals told us that they were able to comment on the service through customer surveys. Three of the people we visited knew how to complain if they were not happy about something, and two of the people we spoke with told us the manager was approachable and regularly checked on the service quality by telephoning customers. The manager told us that they believed they provided a good service because they supported a small number of individuals.

During an inspection in response to concerns

As part of our review we visited four people who use the service in their own homes and contacted a further four people by telephone.

All the people we spoke to expressed satisfaction with the service they received from the agency. They told us that they had confidence in their care workers whom they found to be knowledgeable, capable and courteous. People reported that care workers showed good attention to detail and it was felt that care workers had appropriate skills to meet their requirements and offer them consistent care. We heard that people found their care workers to be mindful of issues around privacy and dignity which enabled them to feel more relaxed when receiving support with their personal care routine. People told us that, in their experience, the service ran smoothly and they had no concerns about the care they received from any of the care workers who visited them.
